A geriatric care manager is a professional who helps families coordinate care, navigate aging-related decisions, and reduce risks during health transitions. At GeriSafe Consultants, services are led by a Registered Nurse with extensive experience in geriatrics and rehabilitation.
Do you provide hands-on medical care?
No. Services are consultative and coordination-focused. We do not diagnose, prescribe, or provide direct medical treatment.
Do you replace my parent’s physician?
No. We support communication and organization among providers but do not replace medical professionals.
Is this covered by insurance?
No. Services are private-pay. This allows for personalized, responsive, and flexible support.
When should we contact a geriatric care consultant?
Common times include:
After hospital discharge
Following a rehabilitation stay
After a fall
When aging at home becomes unsafe or unclear
When family coordination becomes overwhelming
Do you serve all of Connecticut?
Yes. Services are available statewide through a hybrid virtual and in-home model.
